On-Line Claim Form

1. Enter each investment that you owned as of July 4, 2008 and for each new investment or reinvestment you made on or after July 5, 2008.
2. Please enter each investment separately, do not combine investments.
3. Please properly declare all interest payments you received on or after July 5, 2008.
4. Please only submit claim forms if you are the principal investor.
5. Attach documents supporting your claim.
6. Inaccurate forms and claims may result in a denial of your claim.

1. 2008年7月4日時点に存在した投資及び、7月5日以降の投資・再投資についてのみ入力してください。
2. 一つの投資ごとに入力をし、複数の投資口を合算しないでください。
3. 2008年7月5日以降に受け取った配当を正しく入力してください。
4. 筆頭名義人のみ債権届出をしてください。
5. 根拠となる裏付け資料を添付してください
6. 不正確な申告の場合、配当されない場合があります
